Care is one of the many forms of Inner Jack and one of the two positive personalities.
Biograthy
Care appeared to Samurai Jack as a reflection from his post traumatic stress disorder look. He obviously cares about Jack's well being and asked him what he was going to do, he later disappeared after Jack and Ashi were getting ready to eat dinner.
Appearance
Care looked a lot like Jack during the time Jack was teaching Ashi what was really going on in the world of Season 5 and before Episode XCVIII where Jack permanently sealed away Mad Jack.
